The special effect is essentially a Health Link affix for players, splitting unsaved damage taken between all players and followers within 100 yards who are also wearing Unity rings. The damage reduction is determined by the target who takes it, so other linked targets cannot use their own protection against this damage. If either linked target is completely immune to damage (or at least to that elemental damage type), they will not suffer any unsaved damage, and will not transfer any damage to others, but will still prevent part of the damage inflicted upon others. Dead / incapacitated targets may not take part of the damage on themselves. Remember it works two ways, so the player can be killed by the damage their ally is taking.
However, taking Unity for both the player and a follower with the Legendary follower item that prevents them from dying (Enchanting Favor, Skeleton Key or Smoking Thurible) will effectively double the player's own Toughness. In endgame, it can serve as a very powerful survival boost, and rivals other endgame rings. However, this trick will not work in multiplayer. Alternatives to this combo include rings like Justice Lantern, Band of Might, Elusive Ring etc.
