They require character level 70 to drop, and can only drop at Torment difficulty. Only Necromancers can equip them.

Though his fate is tied to that of Sanctuary, none have seen the guardian Trag'Oul for years beyond count. The implications are dire, but the Priests of Rathma have not lost faith.
