Starsign is the first of the storyline quests released with the Terror's Tide update in Diablo Immortal. To unlock it the player must have reached level 60, finished the Forgotten Nightmares questline and changed the difficulty to at least Hell I.
Starsign is immediately succeeded by Terror's Tide.

- Deckard Cain: It's good you're here. Exquisite timing, as always... I've been trying to gather allies to assist us against Hell... and I've finally met with some success. Do you recall the friend I mentioned?[1] He should be arriving today.
His name is Karshun — he is an accomplished mage from Xiansai. I met his teacher in far-off Kurast when we were on the trail of the Great Evil Mephisto,[2] and... well, I suspect he'll be a great help to us both. Could you find him at the docks and show him the way to my shop
- Head to the docks
- Class: Rehm, what happened?
- Captain Rehm: We just disembarked the Black Bower, and the guards' all but shut the docks down. Apparently, a few sea beasts made it aboard. Some outlander with a staff said he was going to deal with them.
- Clear the Black Bower of it's aquatic infestation
After that, talk to Karshun
- Karshun: At least someone in the city isn't paralyzed by fear. Let's find the source of the danger.
- After killing the sea beasts
- Karshun: If the stars have aligned above you as I suspect, we are fated to serve one another. Did you ensure all the beasts were dead?
- Class: Yes. I've done this sort of thing before.
- Deckard Cain: There you are!
- Deckard Cain: Ha! You've gone grey, young man.
- Karshun: Not nearly as grey as I should, Elder.
- Deckard Cain: Welcome to Westmarch, my friend. And not a moment too soon. How was your journey?
- Karshun: Uneventful, unlike theirs.
- Deckard Cain: By the way of introductions: this is Karshun, formerly of the Yshari Sanctum. He has spent much of his life studying the cults of the Great Evils, and vowed to do everything in his power to prevent Diablo's return
- Class: It's nice to make your acquaintance, Karshun. We could use your help.
- Karshun: That's apparent. Since the threat is at bay, you should see what the sailors know, alert the guard, then join us. I'll prepare a divination in Cain's workshop.
- Inquire about the incident from Captain Rehm
- Class: Any idea how those monsters got aboard your ship?
- Captain Rehm: We found some bodies floating north of the gulf. It wasn't right leaving them there... so we brought them aboard for burial. The monsters must have hidden in their... ugh... Just glad you put them down.
- Head over to the palace gates, Commander Kaya should be stationed there
- Class: Commander... I've just come from the docks. There were monsters aboard the Black Bower. They attacked Captain Rehm's passengers before we stopped them.
- Commander Kaya: The Black Bower, you say? A shame... but it sounds like you have it more or less under control. I'll double patrol by the docks, just in case.
I'd appreciate if you kept things quiet. The city hates a panic. - Class: There might be a reason to panic. I should see how Cain and Karshun are faring.
- Meet up with Cain and Karshun at the former's workshop
- Karshun: No time to waste. We will peer through the stars, and relate what we see. You are only to observe — touch nothing.
Astral Realm[]
- The player appears in a castle grounds with cultists and soldiers battling around you
Karshun is missing. Find him through his voice that can be heard in the distance
- Karshun: I rarely divine with such clarity. I don't know where this is — but it's happening right now.
- The player have reaches Karshun. Talk to him
- Karshun: Look around us. Search for details. The architecture, the armor of the soldiers... similar to the guards in Westmarch. Where are we?
- Bride of Hell: (To her companions)The Lord of Terror has heard your promises. He will not forget them.
Take Stormpoint, find the shard, and you will join me by His side.
- Bride of Hell: Hold. I don't know how... but we are being watched beyond the veil.
- Karshun: There is... an odd aura around you. What have you —
- Karshun: Agh — we need to flee. Through the gate!
- Karshun opens an Astral Tear
Deep within the Astral Realm[]
- Karshun: She... sundered our astral connection. How? We — ugh — need... to get back... before our souls split from our bodies.
Not even a practiced Taan mage could see through my projection. Something about your soul shines like a beacon here.
- Cultists are projected by The Bride to stop Karshun and the Shard-seeker
- Bride of Hell: (disembodied) Whatever rock you hide beneath, I will find you.
- Karshun: Hold them back. I will tear through. Connect us to the anchor myself.
- Defeat the cultists and protect Karshun while he prepares an Astral Tear
- Karshun: The Tear is ready. Go!
- Follow Karshun through the Tear.
The Tear leads to another location, still in the Astral Realm
- Karshun: Not far enough. We... we can't be trapped here.
- Bride of Hell: (disembodied) All creation is Diablo's claim.
- Karshun: Then why doesn't he have it yet?
- Defeat the cultists and protect Karshun while he prepares an Astral Tear
- Karshun: We can't let them follow. I've opened another tear. Move!
- Enter the Astral Tear.
It leads to another point still in the Astral Realm. More cultists are projected in
- Karshun: We're getting closer... but the enemy is still at our heel.
- Bride of Hell: (disembodied) I have an army of lives to spend.
- Class: Karshun, they are not stopping. Can you send us further away?
- Karshun: The Tear is already opening. A few moments more.
- The tear is ready
- Karshun: The tear is ready. Go.
Back at Cain's Workshop[]
- Karshun exits the Astral Realm. He is injured
- Class: Karshun!!
- Karshun: (taunting Ifriss)You were just fast enough.
- Deckard Cain: By the Highest Heavens! That was... quite a return trip. What did you see?
- Karshun: A Worldstone shard... is... in a place called Stormpoint. Invaded by an army of Diablo's cultists. Find it... before they do. Nothing else matters.
- Deckard Cain: You're hurt! Come. I'll get you to a healer.
We'll inform the guard. Friend, if Rehm is able to sail, you should make for Stormpoint immediately. Wherever it is.
- Head over to the docks
- Captain Rehm: What's your destination? We may have a stop there.
- Class: Captain Rehm — I need to go to Stormpoint. As quickly as you can. Do you know it?
- Captain Rehm: Not officially... but yeah, for you, I do. It's not far off from that last voyage of mine... I should be able to scrape up a scant crew, but I'm not sticking around after I drop you off.
- Class: I owe you a favor.
- Captain Rehm: At this point, I'd say you owe me several.
