The Necropolis of the Firstborn is a structure near Kasama, located on the other side of the Black Lake.

The depths of the Necropolis
The Necropolis of the Firstborn dates back to antiquity. A trove of magic and knowledge in the time of the Firstborn,[1] the first humans were later laid to rest here.[2] It later became the abode of Rathma.[3]
During the Great Enmity, Inarius entered the Necropolis. Having learnt of Rathma's Prophecy, he confronted his estranged son, demanding a key that would open the Gates of Hell. Rathma refused, and Inarius slew his own son with his own staff, unaware that the key was located within.[3]
Balance's Sanctum
Sometime later, in 1336, the Necropolis was entered by Lilith for the same reason as Inarius. She found her son's body and obtained the key from his staff. Either on the way in or way out, Lilith's hatred manifested as a living and oozing corruption covering the necropolis. When the Wanderer arrived much later, they were forced to destroy the tumors that were the source of the growth before defeating Lilith's Lament. Throughout their passage, ghostly echoes of the confrontation between Inarius and Rathma played out before them. The Wanderer finally made their way to Rathma's personal sanctum, where they found his corpse.[3]
In-game[]
The Necropolis is a Fractured Peaks dungeon in Diablo IV. It only appears in the game's primary questline, and cannot be revisited.

- If the player enters the Necropolis as a Necromancer, they will comment that the structure "feels like home." This is a possible reference to Rathma's role in the founding of the Priests of Rathma and/or the Necropolis, which is also a subterranean structure.
