Kyoshiro's Blade is a Legendary Fist Weapon introduced in patch 2.4 of Diablo III. It requires Character Level 70 to drop.
The unique affix has two multiplicative damage modifiers: the second one triggers if the cast hits few enemies. This only counts the initial instant damage, not other effects. As for Wave of Light, damage of all 8 waves will count. For other Runes, damage of secondary effects will not be counted towards the 3-enemies-limit.
