Izilech the Misshapen is the final Helliquary quest for the Wrathborne Helliquary, succeeding Twin Terrors. Completing this quest unlocks the Voidwound Helliquary, and Hell V difficulty.

Transcript[]
- Rayek: Where the hell's have you been?! The Helliquary's power is failing and the Einfrinn Tree is buckling within itself. The tree... it's dying.
- Class: What's causing this?
- Rayek: It's the Hell lord, Izilech! He's finally making his push through.
This is it. Either he breaks into our world and destroys everything, or you put an end to the Wrathborne. I hope you're ready. - Class: I am prepared.
Use the Helliquary to scry Izilech. It projects his image[]

- Izilech the Misshapen: Your world is mine!
- Rayek: You may have defeated his knights, but Izilech still has his legion. Take your strongest allies with you and do whatever it takes to stop him.
Use the Einfrinn Tree to go to the Hall of Fangs[]

After the reinforcements are killed, Izilech returns to fight the player[]

After that, he rears his bow back for a swing, and the ground beneath the player glows with hellish flames[]

- Izilech the Misshapen: Do not flee, cower.
Izilech strikes the player's location with his bow. Luckily, they freed themselves and escaped in the nick of time[]

- Izilech the Misshapen: Fall in piles.
- Izilech the Misshapen: Terror itself birthed me, changed me, gave me my hunger.
Izilech returns to his throne, and his army holds a defensive line in front of him[]

Take the Wrathborne Crown and leave the Hall of Fangs[]
- Rayek: So you've done it. The Wrathborne are no more.
- Class: With the Einfrinn Tree secure, perhaps you can finally get some rest, Rayek.
- Rayek: Perhaps... but the Helliquary has more potential than I could ever imagine, and I've already thought of some improvements I'd like to prototype... I just want to build out a few schematics while they are still fresh in my mind. Then I can rest!
