Belphegor was a servant of Diablo. During the Eternal Conflict, she commanded Diablo's siege demons in an attack against the Pandemonium Fortress.[1] After the disappearance of the Worldstone and stalling of the Eternal Conflict, she apparently remained in Pandemonium.

Aeons later, she was confronted by the Nephalem, who slew her to obtain a siege rune.[2] In the resulting explosion, all that remained of her was a single hand.[1]

In-game[]

Belphegor, Siege Runekeeper is a Super Unique Warscarred Ravager found in Path of War in Act V during The Battle Fields of Eternity quest in Diablo III. She attacks alongside a horde of Marauders, Ravagers and Firewings, and in addition has Plagued, Thunderstorm and Jailer affixes.

Upon death, she drops the first of the three Siege Runes the player needs to activate the Ram, and allows talking to Imperius to continue the quest.
