He spawns as random Unique with Flayer and Flayer Blowdart minions upon activating an altar in Flayer Jungle ruins, while doing the Blade of Old Religion quest. The Unique Rat Man drops The Gidbinn. While it's a random Unique, this monster is more than that by being a special quest variation of Fetish.
Weirdly there are no Fetish codenamed fetish9 and fetish10.
