Blood petals are red petals that were were associated with Lilith and her followers.
Lore[]
Blood petals appeared alongside Lilith upon her return to Sanctuary during the Great Enmity.[1] The petals were subsequently left behind by her in various locations across Sanctuary.[2][3]
Of note, the villagers of Nevesk (who had been enthralled by Lilith) used blood petals in sacrificial rituals. One intended sacrifice was the Wanderer who was force-fed the petals, but was saved by Iosef.[4] As the Wanderer pursued Lilith across Sanctuary, the blood petals left behind by Lilith allowed the Wanderer to view past events concerning the demon.[2][3]
Lilith's blood petals remained in the mortal realm even after her death. An enigmatic figure arrived in Estuar from the west, collecting Lilith's blood petals as they made their way across across the continent. To what end was only known to them.[5]
In-game[]
Blood petals appear throughout the storyline of Diablo IV. Interacting them will trigger a cutscene, specifically a flashback involving Lilith, depicting her actions in the area where the blood petals are encountered.

- The flavor text of Blood Petal implies that the blood petals have latent magic within them.
- Blood petals pulse when they touch the Wanderer's skin. According to Voszalko, it was proof they were sullied by Lilith.[6] This 'proof' is dubious however, given Voszalko's motivations.
