Bilefen Island,[1] more commonly just called Bilefen, or "the Bilefen",[2] is a jungle island[3] located just south of Aranoch.[4] It is frequented by pirates and traders alike, though none extend their stay.[5]
Lore[]
Murky waterways run across the island,[6] and the island is dotted by ancient abandoned temples.[7] Altruism is rare on this island.[8]
History[]
In ages past, Bilefen was inhabited by the nephalem[9] as part of the kingdom of Valostre.[10] Due to the actions of Sargoth, Valostre fell into ruin.[11]
At some point, humans arrived on Bilefen.[8] Later, Westmarch established a number of settlements[12] This was apparently long after the initial human settlement.[8]
When the Worldstone was destroyed, one of its fragments landed here, making a bad situation worse for those who lived on the island.[13]
By 1270, a plague was spreading across the island, with Plague Carriers serving as the apparent vectors.[14] Human and beast alike succumbed to it, and when the infection had run its course, the victim was left an empty husk.[15]
Also by this year, the island's Fetish were attacking the island's settlements,[12] including Port Justinian itself.[16] Demonic destruction flooded the island's waterways, grinding daily life to a sludgy halt.[6]
In-game[]
The island appears as a zone in Diablo Immortal.[17] It has a recommended Character Level of 40-45.
Playing through the storyline of Immortal, players will reach it from the Library of Zoltun Kulle. A terminus will deposit them on the eastern side of the island. Alternatively, they can use Captain Rehm, who will deposit them on the western side of the island, close to Port Justinian.

Trivia[]
- While it is not outright stated, it appears that the human populace of Bilefen hails from Westmarch, and has settled the island. Port Justinian shares the name of the Justinian Dynasty, and Elder Owen refers to Akarat—an important Zakarum figure, which is Westmarch's religion. Similarly, the humans of the island appear ethnically similar to those of Westmarch.
- Similarly, in the side quest "A Season of Charity," it is heavily implied that Bilefen has had at least two waves of human settlement, as Alaw mentions helping people on Bilefen "long before" the coming of Zakarum. While it is possible that the pre-Zakarum human population were descendants of the nephalem, there is nothing to suggest this is the case.
